As PMC clinical teams grow and develop into a more comprehensive team than they had been initially, it is imperative to address change. We have seen and been part of both easy and difficult team transition and growth. Having seen the successes and the trials, we want to provide a pathway for optimized team development and expanded inter-professional team patient care. Executive Directors, RN's, RDMS's and Client Services Directors will all benefit from this panel of PMC team members sharing how they work together and avoid collisions as they serve their communities. It's not always easy serving in a stressful environment. Making a way for all to answer the call to serve those at risk, takes being aware and alert to what each profession is expecting. This session should serve as a strong foundation for inter-professional team development and optimized collaborative care, resulting in beneficial patient outcomes.
